SIGH Information Forms ("Qdoc") Instructions
Please be sure you understand these instructions before filling out the "Qdoc" forms.  Email Lin Jenkins or Gayle Nastasi with any questions you might have.

Web Site Instructions
Follow, in order, the links at the bottom of this page.  Begin with Part I of the SIGH Qdoc.
 
  1. Wait for the Qdoc web page to load completely.
  2. When the page has finished loading, click the print button on your browser.  Wait for the page to print.
  3. When the first portion of the Qdoc has printed completely, click the Back button on your browser to return to this instruction page.
  4. Select the link for Part II of the SIGH Qdoc.
  5. Repeat the print process for the second portion.

Mailing Instructions
  1. Fill in the SIGH Qdoc to the best of your ability.  You may need to consult your veterinarian for some of the information.  Please answer as many of the questions as you can, but if there is some information you cannot obtain, please send as much as is available to you.  Even partial documents are valuable.
  2. Collect additional material such as test results, pedigree, photograph, necropsy reports, and attach them as per instructions on the Qdoc documents themselves. Staple all attachments securely. It is preferable to include photocopies of printed documents rather than the originals. Photos will be returned upon request.
  3. If desired, make a photocopy of the completed Qdoc forms to keep for yourself.
  4. Gather THREE envelopes:  one large envelope and two smaller envelopes. (Recommended: one 8-1/2 by 11 manilla envelope and two 5 by 7 manilla envelopes.)  Utilize them in the following manner:
    1. Insert the IDENTIFYING INFORMATION section of the Qdoc (Part I) into one smaller envelope.  Include any additional materials specified on that section.  On the BACK of that envelope, write clearly:  IDENTIFYING
    2. Insert the MEDICAL INFORMATION section of the Qdoc (Part II) into the other smaller envelope.  Include any additional materials specified on that section.  On the BACK of that envelope, write clearly:  MEDICAL
    3. DO NOT address these smaller envelopes.  DO seal both of these envelopes securely.
    4. IF YOU ARE FILLING OUT MORE THAN ONE SET OF FORMS, please add an identifying letter to the markings on the outside of the small envelopes.  For example, if you are filling out forms for two dogs, mark the small envelopes IDENTIFYING-A, MEDICAL-A, IDENTIFYING-B, MEDICAL-B, taking care to match the letters so that the medical half and the identifying half can be properly attributed to the correct dog.
    5. On the LARGE envelope, write your return address to avoid documents being lost in the mails, and address that envelope to your choice of the individuals listed at the bottom of this web page.
    6. Insert BOTH of the smaller envelopes into the large envelope and seal securely.
    7. Affix proper postage and mail.
